Her expertise with minimum data set coding helps clients improve patient care, regulatory compliance, and financial outcomes for services paid under Medicare and Medicaid. If you are using these forms and using a mobile device to fill out your CMS-672 form be certain to print a copy of the form and attach it to your CMS-672 form. You MUST attach a copy of your original birth certificate (a copy with the photo) to the CMS-671 MDS form as well as your US passport if applicable. CMS-671 CMS-678 For more information regarding the CMS-672 and CMS-678 forms please see the information below: CMS-682 The CMS-676 MDS (MCD) is not for use by persons who live in State Government housing, and therefore the CMS-676 forms are not used for that census.
