Application Form. Complete the Application for Employment as an Independent Contractor Form, Form CMS-116, with the proper fees and the necessary documents attached as indicated on the form. What is the purpose of Form CMS-116?
This section provides an explanation of the purpose of the CMS-116 form and how it is used. To ensure accurate eligibility determination. To protect the privacy of eligible individuals. The CMS-116 form consists of the following six (6) sections: Eligible Individuals The first section of the form, entitled “Eligible Individuals” provides information regarding an individual's identity, citizenship, immigration status, sex, age and place of employment. The Privacy Act also requires that the information collected in this section is protected and not revealed to a third party. This section does not require that the individual provide access to personal information about himself or herself but rather must be presented solely for eligibility verification purposes. The information contained in this section includes: name, date of birth, gender; marital status; educational qualifications including date of enrollment, major, grade level and area-of-study awarded; employment records; bank account information; credit or debit card numbers; and any other information as may be necessary for eligibility verification, including name change, date of birth verification, etc. The second section, entitled “Applicants” is used to verify information required by the federal law called the Immigration and Nationality Act (INA). This section refers to the application package as a whole and does not require an individual to provide any personal information. Information and information in the name section should only be collected to verify information on the form and the immigration status. For immigration information, the applicant's name must be verified. For employment information, the applicant must provide documentation (not including a copy of the application package) indicating the employment history of that individual. The third section of the form contains the citizenship and immigration status of the applicant. The fourth section, entitled “Other Information” is for verification and verification purposes only.
